One-to-One Consultations: Face to Face and/or Online

Whether you're looking for a one off consultation or a package I'm confident I can meet your needs.
What to expect....
Before your consultation
To get the most out of your consultation you will be asked to complete:
A Getting To Know You Form requesting information such as your GP details and Past Medical History. I may also ask you to have some bloods done (for example HbA1c, or cholesterol)
A Food Diary so that I can get a really detailed idea of your diet.
During your consultation
We will talk through your condition, medications (if you're taking any) any relevant blood results and I will explain what everything means. If you have a continuous glucose monitor we can spend time reviewing your patterns. I can help you to make sense of your readings and how to get the results you want!
We can talk about how your condition affects you, any specific concerns you have and you will have the opportunity to ask all the questions you need.
In addition to the information you kindly provided before the consultation I can offer specialist advice on managing your condition.
If you select the Core Package we will cover everything you need to know to take complete control of your diabetes whether you're diet controlled, take oral medications, insulin or have a pump. Together we'll create a plan that's truly personalised to your needs.
This may also include estimated dietary requirements.
Afterwards
After your appointment I will complete a full written report, including any agreed goals and supporting information which will be emailed to you within 3 working days.
I will also be happy to contact any healthcare professionals who may also be involved in your care if you explicitly consent to this.
If you need a follow up consultation or have selected a package we can arrange this at your convenience.
